The Vermont Case Management Project

Improving Case Manager Competencies

When Vermont passed Act 147 in May 2000 (“Assisting Families to Attain Self-Sufficiency”), PATH developed the Reach Up program to help Vermont’s families become attached to and remain in the workforce. Although PATH staff were providing case management services prior to Act 147, provisions of the Act called for delivering even more comprehensive case management services to every family receiving Reach Up financial assistance, adhering to shorter timeframes for moving adults into countable work activities, establishing realistic employment goals with adults, supporting the development of employment goals for adults, supporting the development of skills and strategies to help secure and retain employment and managing larger case loads.

The overall goals of the performance-based training program include knowledge, skill and attitudinal development in the areas of:

  • Assessment
  • Financial Planning and Forecasting
  • Core Functions of Case Management
  • Career Path Development
  • Coaching and Mentoring
  • Collaborative Problem Solving
